Three community-based, charitable non-profits come together to meet the demands of the Evolving Health Care Environment. Each share the goals of providing services that increase quality of life and enable elderly and at-risk adults to live independently in their own homes.
Town & Country Support Services: Since 1978 - 33 years
“We continue to be committed to the communities we serve and the residents of Huron and Perth, offering programs in local communities. People will continue to receive the same service in the same place from the same people but there will be opportunities for us to develop and integrate to serve people better over time. · 250 staff; 503 volunteers serving 3805 clients in two Counties.
· Thirteen programs designed around Elderly care including in-home Personal Support, Wellness, Nutrition, Transportation, Reassurance and Home Help.
· Four office locations in Huron and Perth Counties: Wingham, Clinton, Exeter, Stratford and programs through the two counties.
· Budget of $6.3 million, with funding through the Community Care Access Centre for personal support services and the South West LHIN for community services, client fees, grants and donations.
Midwestern Adult Day Services: Since 1977 – 34 years
“At its core, this is about our enhancing our mission to provide service. This opportunity will allow the agencies to package and provide a range of programs which will continue to meet the needs of a growing population – our seniors and adults with disabilities living in Huron and Perth Counties.” - President, Board of Directors – George Gibson
· 34 staff, 161 volunteers serving 247 clients in Huron, Bruce and Lambton Counties.
· Adult day centres offering social, recreational and health related programming for frail seniors facing the challenges of aging; older adults coping with the effects of one or more chronic diseases such as stroke, heart disease, Parkinson's Disease, Multiple Sclerosis, acquired brain injury; and those with dementias such as Alzheimer Disease.
· Five program locations: Clinton, Exeter, Grand Bend, Wingham and Goderich.
· Budget of $1.35 million, funded through the Southwest Local Health Integration Network, client fees, donations and grants.
Stratford Meals on Wheels & Neighbourly Services: Since 1969 – 41 years
“As Home and Community Support Service agencies, we are committed to strengthening individuals’ ability to live in their own home and community and to do that as part of the health care system. This will help us to provide a broader range of services and simplify access for other health care providers and for our clients.” - Chair, Board of Directors – Ollie Henry
· 18 staff, 500 volunteers serving 1671 clients in Perth County.
· Supporting seniors and people with disabilities in an active, healthy, and independent lifestyle by offering a variety of nutrition, transportation, wellness and friendship services that allow clients to stay in their own homes.
· Serving the City of Stratford and surrounding area. · Budget of $1.2 million, funded by the Southwest Local Health Integration Network (LHIN), the United Way, client fees, grants and donations.
