Services

Introduction

Town & Country Support Services provides a wide range of effective community and home support services to help seniors and persons with disabilities in Huron and Perth Counties live independently in their homes. This support also helps family and friends in their role as caregiver, and fosters strong, supportive communities. Services are provided by professional staff and dedicated volunteers who recognize your wish to remain in your chosen place of residence as long as you are able.

Dining for Seniors

Enjoy a hot meal and conversation with one of our weekly or monthly programs held throughout Huron County. Second helpings are always available for hearty appetites. There's also time for visiting, entertainment and social activities. Volunteer transportation is available for those who need a ride. Programs are available in the following locations:

  • Exeter Lions Youth Centre every Tuesday at noon
  • Hensall United Church the 3rd Thursday of each month (except for July and August) at noon
    • Zurich Arena every Wednesday at noon
    • Grand Bend, St. John's By The Lake Anglican Church, the 2nd and 4th Thursday of each month at 5:30 pm
    • Clinton, Betty Cardno Centre every Wednesday at noon
    • Seaforth Arena the third Thursday of each month at noon
    • Wingham, Midwestern Adult Day Centre, every Friday at 5:30 pm
    • Teeswater United Church every 2nd and 4th Thursday each month at 5:30 pm
    • Blyth, Trinity Anglican Church, the 1st Friday of each month at noon
    • St. Helens Community Centre the last Wednesday of each month at noon
    • Gorrie United Church the 3rd Thursday of each month at noon
    • Brussels United Church the 2nd and 4th Thursday of each month at noon
    • Friendly Visiting and Telephone Reassurance

      Friendly Visiting creates lasting, meaningful relationships between seniors and volunteers. With regular friendly visits–in person and/or over the phone–isolated seniors experience improved physical, mental, and emotional health, as well as gain a more positive outlook. Time is given for conversation, activities, and good companionship. This service is designed to provide seniors with regular social contact by a personal home visit from a caring volunteer who is matched to each individual. For the Telephone Reassurance program, a caring volunteer will make a phone call to people living alone to ensure that all is well. Telephone calls are established at a mutually agreed time, daily or otherwise. This is a good way to keep in touch and provide an element of security.

      Frozen Meals

      Nutritious meals ready to heat at your convenience. Choose from a tempting menu of entrees, soups and desserts. The menu features a wide variety to suit your tastes. Special diets can be accommodated. Meals are delivered every two weeks throughout Huron County. Meals can be easily heated in the microwave or conventional oven.

      Home At Last (HAL)

      HAL provides help to adults 55 and older to settle in at home after being in the hospital. HAL identifies seniors in local hospitals in Huron and Perth Counties who have limited supports upon discharge or potential concerns regarding how well they may settle when they return home. Upon discharge from hospital, transportation services are available to get the client safely home and a Personal Support Worker (PSW) will stay with the client for up to four hours. Once home, the PSW will do a safety check of the home, and provide personal care and homemaking services. A complimentary meal is provided, and if needed, the PSW can pick up some groceries and/or prescriptions for the client. Time is given to answer questions and to review support agencies available in the community to meet the client's particular needs. Clients also receive a follow–up call within 48 hours to ensure the client is managing well.

      Home Help

      This program provides assistance inside the home with housekeeping, shopping and meal preparation. Home Help also provides assistance with special cleaning, window cleaning, lawn and yard care, spring cleaning, and fall yard clean up. Work done is individual to the person's needs. Qualified workers can assist with tasks around the home to help make your home safe and comfortable. This fee-based service is available to anyone looking for extra help around the home.

      Homemaking

      A professional Home Support Worker will assist with activities of daily living and will help to:

      • Establish household routines
      • Teach homemaking skills
      • Adapt the home to meet the client's needs
      • Help with money management and budgets
      • Dust and vacuum
      • Laundry and dishes
      • Clean the kitchen and bathroom
      • Clean appliance, such as the fridge and oven
      • Escort clients to medical appointments
      • Help run personal errands such as going to the bank and drug store
      • Encourage participation in the community
      • Lifeline Emergency Response

        Life is unpredictable. An unexpected fall or medical emergency can happen at any time, leaving you cut off from the help you need. But with Lifeline, you're never alone — prompt, expert help is available around the clock at the push of a button. Lifeline is an easy–to–use personal response service that lets you summon help even if you can't speak. All you have to do is press your Personal Help Button, worn on a wristband or pendant, and a trained Personal Response Associate will ensure you get help fast. Lifeline provides peace of mind for you and your family and enables you to live independently and confidently in your own home.

        Meals on Wheels

        Enjoy a home–style, nutritious meal delivered right to your door by Town and Country volunteers up to five days a week. Meals on Wheels provides an easy and convenient substitute for meal preparation. The meals are affordable and having a hot, nutritious meal has been shown to contribute to an individual's recovery following illness or hospitalization. Most special dietary needs can be met.

        Personal Support

        A certified Personal Support Worker will assist with personal care needs, such as:

        • Bathing
        • Mouth Care
        • Shaving, shampooing, and dressing
        • Skin care and grooming
        • Toileting, transfers
        • Assist with prosthetic device
        • Child care and respite care
        • Help with mobility
        • Assist with special care as directed by health professionals
        • Assist with eating
        • Respite

          Caregivers who provide constant care to a frail or cognitively impaired senior (or a person with a disability) i n their own home need time away from the caring situation. The respite service allows caregivers to take a break from their responsibilities and have some time for themselves, while knowing that their family member is being well–looked after in their own home.

          Social Programs

          Enjoy time with friends over a cup of coffee and a game of cards or crokinole at one of our social programs.

          • Euchre is held every Monday at 1 pm and crokinole is held every Friday at 1 pm at the Betty Cardno Centre in Clinton
          • Crokinole is held every Tuesday at 2 pm at the Maitland River Community Church in Wingham and Coffee Break is also held at the church every Thursday at 10 am
          • Solo is held every Wednesday at 1 pm at the Exeter Lion's Youth Centre
          • Carpet Bowling is held Mondays and Fridays from 9–12noon at the Exeter Lion's Youth Centre
          • Coffee Hour is held the 1st Wednesday of each month at Hensall United Church and the 3rd Wednesday of each month at the Lion's Youth Centre.
          • Senior Luncheon is held the 1st Monday of each month at the Lion's Youth Centre, Exeter
          • Mah Jong play and instruction is held every Thursday at 1 pm at the Bayfield Lions' Building.
          • Supportive Housing

            Provides on–site support services to aid residents living in specific buildings — 85 West St. in Goderich, Maplewood and Blue Water Rest Home Apartments in Zurich, Heartland Apartments in Clinton, and Highland and Maple Villa Apartments in Brussels. This program helps residents with their daily activities so that they can continue to live independently in their apartments by providing them with essential services such as personal care, light housekeeping, emotional support and monitoring, and a timely response to emergency activities. Social programs are held so individuals remain active and involved.

            Transportation

            Transitioning from the driver's seat to the passenger seat is not easy for older adults, but navigating this major life change is easier when available transportation options keep older adults in control. Our transportation programs help older adults remain mobile once they limit or stop driving allowing them to remain connected to the families, friends and activities that bring meaning to their lives. Experienced volunteer drivers provide transportation to and from medical appointments, personal errands and social activities. Fully accessible van services offer transportation for people with mobility limitations. We provide door-to-door service that is affordable, safe and reliable for both local and out-of-town trips. We offer services for registered clients who are seniors, as well as for clients who:

            • Are without access to transportation or are where public transportation is not available
            • Require specialized transit, e.g. wheelchair access
            • Do not have family and friends who are able to help out
            • Eligibility requirements will apply
